Thankyou for taking part in our survey, on your flyer you will find a promo code and five questions, please type in your promo code at the top of the comments box, then answer each question on the card with a number 1 to 5 as to how you feel about the subject.
1 being not at all worried and 5 being Its my top concern.
Example:
1. Do you worry about home security in youre neighbourhood (if you are very
worried you may put a 4 or even a 5.
2. Are you worried about youth crime in your area. ( you may not be so you
would type 1.
Just put youre numbers down in a list seperated by commas in the comments box.
Example:
Promo code 12345 (promo code from flier)
4,5,1,3,2 (your list of answers)
And thats it, If you have provided a valid email address then we will contact you with details of the next draw and any other relevant information.
